Direct Support Provider - all NH regions

Summary:

We have full-time and part-time position available throughout New Hampshire and Eastern Vermont providing home and community support for individuals with developmental disabilities. The positions include both day and evening hours, and some weekends. Candidates must be flexible, possess a clean driving record and pass a criminal check. We prefer candidates with prior experience, but will train.

Duties/Responsibilities:
May perform one or more of the following:

1. Supervises and transports our consumers during assigned hours, this may include job search, medical appointments, weekly recreation, shopping trips, and other necessary trips.
2. Supports and teach consumer new skills specific to goals stated in Individual Service Plan.
3. Provides a positive, supportive role-model for the consumer.
4. Reinforces appropriate consumer behavior through direct feedback, role-playing.
5. Assists consumers with self-care
6. Documentation and timely submission of all progress notes, weekly summaries, attendance and all other consumer documentation deemed necessary by the company.
7. Attends trainings and staff meetings.
8. Other duties as assigned.

Qualifications:
Clean Driving Record and ability to pass a criminal check. We prefer candidates with prior experience, however, we provide comprehensive training.
